I have a consult case! Wants to know your thoughts...33 yo M right testis 9.5x7 cm well circumscribed mass limited to the testis. Spindle cell tumor with 2 mitosis/10 hpf. CT negative for LN mets. IHC positive for sex cord stromal tumor markers(inhibin, calretinin focal, SF-1 and FOXL2), SMA, focally positive for s100, dot-like pattern positive for panCK and CAM5.2. My ddx includes fibrothecoma, sex cord stromal tumor unclassified and myoid gonadal stromal tumor. Any suggestions? Thanks!

@赵明 浙江省 @贺慧颖 北大医学部 Yes! I signed it out as unclassified SCST. Reticulin pattern doesn’t support fibrothecoma.  Per report, about 20% of unclassified SCST behaves in malignant fashion. And fibrothecomas in testis are all benign even for the cases with mitosis is up to 5/10 hpfs. I suggested clinical follow up!
