相似两种癌的形态，但是与表面上皮的关系看来，似乎从下往上累及的可能大些，同意赵老师的：前列腺癌累及吧，是否伴有神经内分泌分化……

Summary of IHC: positive for NKX3.1, PSA+PAP, racemase, pancytokeratin, very focal positive for ck20; negative for GATA3, ck7, synaptophysin, chromogranin, s100.
Final Dx: prostatic adenocarcinoma Gleason score 549 involving bladder urothelial mucosa. No muscularis propria is identified in the specimen. By definition, this should be pT4 prostatic adenocarcinoma. Residual urothelial mucosa without dysplasia. No neuroendocrine carcinoma component identified .
