免疫组化：p16, ki/67,后二张都是ki-67。没有正常宫颈宫内膜组织。没有间质组织的浸润。签別诊断…。

My differential diagnosis 包括Papillary squamotransitional carcinoma VS papillary SCC.世界卫生组织的书上有好的描述。乳头状构，中间是血管间质组。多层上皮细胞与CIN3 相似。

似乎没有看到明显的间质浸润，不是类似尿路上皮癌的生长方式吧

这种生长方式已超出了原位癌

它应该不是原位癌！只是它的上皮细胞镜下像HSIL. Dr. Crum 的书：Diagnostic Gyn and Obs pathology,第316 页有很漂亮的照片。

You can Dx: SCC with papillary squamous feature, if u feel not very comfortable to make detailed Dx in the biopsy specimen. With comment: It is SCC. The papillary squamous Ca is suggested. The depth of stroma invasion cannot be evaluated due the type of the specimen. Detailed type of cancer and stroma invasion will defer to the further specimen. In the way you are safe even if no invasion is present finally (squamous carcinoma in situ is carcinoma also) and the information is good enough to gynecologists for further management of the patient.

@赵澄泉UPMC 谢谢您的指导，乳头状鳞癌大体标本可以是浅表浸润甚至没有浸润。大家把握好诊断标准，以后不用纠结了。
