恶性附壁结节（肉瘤样癌）【中美加卵巢肿瘤病理交流群335】

我贴一例卵巢病变。50岁女患者，34公分囊性病变，CT影像提示良性囊肿。手术标本显示大部分30公分左右是薄壁粘液肿物，4-5公分多囊部位含有3公分实性结节。
大部分肿物如图1，4-5公分如图2和3，实性结节如图4

Mucinous carcinoma with sarcomatoid carcinoma (so called malignant mural nodule) arising from mucinous borderline tumor was the diagnosis. The case was reviewed at TMC citywide GYN conference and it was the consensus diagnosis.
I had 2 questions before the conference. 1. Is it a mucinous borderline tumor with micro invasion and malignant mural nodule? All peoples thought the mucinous epithelia were too much and micro invasion with the sarcomatoid carcinoma was kind of confusing. I agreed with them. 2. Can the pure spindle cell region with minimally positive keratin be sarcoma? People thought it is ok to have very week epithelial markers for sarcomatoid carcinoma.
